Direct Access Testing (DAT) has been around for several decades, with the concept of consumers being directly involved in their own lab testing. Beginning as early as the 1950’s with over-the-counter (OTC) urine glucose and ketone tests. The availability of OTC tests for pregnancy and other biomarkers has been a part of American healthcare for decades, with continued development (1980s-present).
Increased involvement and policy shifts (early 2000’s-2014)
A broader movement for consumers to have more direct control over their healthcare and health information led to policy changes. The Centers for Medicare & Medicaid Services (CMS) and other agencies proposed rules to amend CLIA Regulations and the HIPAA Privacy Rule for individuals to access their own test reports from laboratories.

Regulatory oversight: DAT is regulated at the Federal Level by the Clinical Laboratory Improvement Amendments (CLIA) program, which sets quality standards for all human-based laboratory testing. State laws also vary, with most states allowing DAT under certain conditions and others allowing it without restriction. DAT allowed 70%-80% off retail prices.
